Analysts at Wolters Kluwer Financial Services, Waltham, Mass., a division of Wolters Kluwer N.V., Alphen aan den Rijn, Netherlands, came up with the list by reviewing state market conduct exam results.
Failure to use licensed and appointed producers, or to keep proper track of producer appointments and terminations, ranks second on the list.
