Service Helps With Hospital-To-Home Transition

Alicare Medical Management has set up a telephone-based service that can help patients who have recently left the hospital stay out of the hospital.

Alicare, a unit of Amalgamated Life Insurance Company, White Plains, N.Y., says the new Patient Transition Program will have registered nurses call patients who have recently left the hospital to make sure they understand how to take care of themselves, what kinds of problems might occur, and when to seek medical attention.

When necessary, the nurses may call patients’ doctors to get more information or arrange for follow-up care.

Amalgamated will offer the program on a stand-alone basis or in conjunction with the company’s own health care utilization management programs.

Employers will pay a one-time case rate for each enrolled patient, Amalgamated says.
