Avivia Offers Health Programs

A unit of Kaiser Permanente has started selling a menu of wellness and disease management programs to employers on a stand-alone basis.

Avivia Health, Atlanta, has rolled out Avivia Select.

The program, designed for employers with 5,000 to 25,000 employees, offers three prepackaged solutions for individuals with chronic conditions and health risks.

The programs are available nationwide and can be used with almost any health insurance program, according to Avivia.

All programs include a health risk assessment and a variety of Web tools.

A middle level includes patient telephone access to a health coach.

A third level involves more analysis and segmentation of the patient population, and health coaches who make active efforts to call members who appear to face a moderate or high risk of developing serious health problems.

The cost of the basic level of service starts at about 50 cents per member per month.

Kaiser Permanente, Oakland, Calif., is a health maintenance organization company that helped develop the modern managed care system.
