Anthem Blue Cross and Blue Shield will start selling a New Hampshire HealthFirst health insurance plan to small groups in the Granite State Oct. 1.

Anthem Blue, a unit of WellPoint Inc., Indianapolis, says the plan, part of a new state health coverage program, will have premium base rates equal to 10% of the state median wage.

Participants who participate in wellness and condition management programs can qualify for incentives of up to $200 in the first year and can reduce their deductibles in the second and third years by up to $1,000 per year, according to Anthem Blue.

All participants will have access to a “care navigator,” to help them understand and use their benefits.

Some of the preventive care benefits included in the base package areimmunizations, routine physical exams, annual gynecological exams and family planning visits, annual care plans for members with chronic illnesses, mammograms, pap smears, lead screenings, prostate screening tests, cholesterol screenings and diabetic screenings. Routine hearing exams also are covered in full.
