American Community Mutual Insurance Company has added the Latitude small group health product.

American Community, Livonia, Mich., has designed the plan, which is now available in 7 states, to appeal to employer groups with 2 to 50 employees.

Employers can combine the plan with a health reimbursement arrangement program, and they can cut the base medical premium up to 17% by building a hospital co-payment provision into the plan design.

Employers also can choose the deductible, the benefit percentage, the office visit co-pay and the out-of-pocket maximum.

The main provider network includes doctors and hospitals in Arizona, Illinois, Indiana, Iowa, Michigan, Missouri and Ohio. The plan also provides access to a national travel network.

Some of the other product features are a $5 million medical lifetime maximum; an accident benefit that waives the deductible; a family security benefit; a prescription drug discount card; and preventive care benefits.

Some of the product options available are prescription co-payment plans; 24-hour occupational coverage; accident benefit removal; deductible carryover; and dental, vision, weekly income, dependent life, life, and accidental death and dismemberment coverage.
