Massachusetts Mutual Life Insurance Company is making a free planning document template available for consumers who are caring for people with disabilities.

The template, for a letter of intent, can help family members and other caregivers develop life care plans for individuals with disabilities or other special needs, according to MassMutual, Springfield, Mass.

A letter of intent is not legally binding, but it can provide guidance for future guardians in case a current parent or guardian dies, MassMutual says.

The letter might include the patient’s medical history; emergency contact information; advice about preferred living, education or work arrangements; recreation preferences; behavioral challenges; and family financial information.

MassMutual is distributing the template on compact discs.
