A Northeastern insurer says it will be providing more personalized help for older health plan members.

Health Net of the Northeast Inc., Shelton, Conn., a unit of Health Net Inc., Woodland Hills, Calif., is setting up a “Take Care” health advocacy unit for its 43,000 Connecticut Medicare Advantage managed care plan members and its 11,000 Connecticut Medicare prescription drug plan members.

The unit will provide access to staffers who can help clear up confusion about coverage, registered nurses who can give members advice about chronic conditions, and pharmacy technicians who can help recommend generic alternatives or on-formulary name-brand alternatives to off-formulary name-brand drugs.

A new transition team will help members who enter the hospital and then need assistance with getting home-based skilled nursing care or long term care services.
