Mutual of Omaha Insurance Company, Omaha, Neb., says its new Disability Income Choice Portfolio includes four different disability plans.

By checking boxes, consumers can use the application for the disability plans to apply for additional critical illness and hospital confinement coverage, Mutual of Omaha says.
