Priority Health is marketing a 2-tier managed care program that uses richer benefits to reward employees who try to take care of themselves.

The new HealthbyChoice Incentives program, which offers a menu of 4 health maintenance organization and 4 preferred provider plan options, is available for employers with 2 or more eligible employees, according to Priority Health, Grand Rapids, Mich., a managed care company that provides or administers health coverage for 585,000 Michigan residents.

All employees start with a deluxe level of co-payments and out-of-pocket costs, Priority Health says.

Members and spouses must complete an online health risk assessment and have a doctor submit a qualification form to keep the deluxe level of benefits.

Members and spouses can continue to qualify for deluxe benefits by showing that they are staying away from tobacco, controlling their weight and controlling their blood pressure.

Members who do not meet the deluxe criteria can still qualify for deluxe benefits if they have 2 additional lab tests and follow a health plan recommended by a doctor, Priority Health says.

Plan members who do not meet deluxe plan requirements within 90 days will be transferred into a standard plan that requires higher co-payments and out-of-pocket costs, Priority Health says.
