Florida Blue Unit Adds Insured Individual Dental Options

The policies will protect members who visit in-network providers against balance billing, and members will not have to submit claim forms for services, Florida Combined says.

The choice copayment plan offers a network of 1,400 providers and a fixed out-of-pocket copayment costs for all in-network procedures

The choice plus plan offers a higher benefit level, access to 3,500 network providers, and a cost-sharing arrangement based on coinsurance percentages. Reimbursement for out-of-network care will be based on a percentile of “usual and customary” dental fees, Florida Combined says.
