AXA Equitable Life Insurance Company has developed a package of reference materials aimed at sponsors of small and midsize 401(k) plans.

The Fiduciary Toolkit package offers quick access to pertinent documents and detailed advice on avoiding 10 common fiduciary mistakes, such as not documenting fiduciary decisions and not maintaining supporting documentation related to the plan's investment, administration, monitoring and management, according to AXA Equitable, New York, an affiliate of AXA S.A., Paris.
