New Baltimore Plan Serves Chronically Ill

XLHealth Corp. has set up a Medicare managed care plan for 98,000 Maryland residents who suffer from serious, chronic health problems.

The new XLHealth Plan includes Medicare Part D prescription drug benefits and disease management services along with traditional health benefits.

The plan is open to Medicare beneficiaries with diabetes, congestive heart failure or end-stage renal disease living in the city of Baltimore or in Anne Arundel, Baltimore, Carroll, Harford, Howard, Montgomery or Prince George’s counties.

XLHealth will set up a team that includes a local health care manager, a local field nurse, a telephone coach nurse and a pharmacist for each plan member.

The plan has developed a formulary designed for chronically ill members. It also will provide preventive screenings, home nursing visits, and help with health care devices and equipment purchases.

The plan will try to appeal to members’ doctors by paying them for some services not covered by Medicare and supplying up-to-date patient reports compiled in an easy-to-read format, XLHealth says.
