Pennsylvania Carrier Helps Members Fight Chronic Conditions

Independence Blue Cross is introducing a program that will manage care for members with complicated chronic diseases.[@@]

Independence Blue, Philadelphia, has picked Accordant Health Services, a unit of Caremark Inc., Bethesda, Md., to run the program, which will provide support services and educational services for about 20,000 health plan members and their doctors.

Nurse counselors will help program members find doctors and hospitals and give members information about community resources, such as home health aide services, Independence Blue says.

Conditions included in the program include chronic inflammatory demyelinating polyradiculoneuropathy, amyotrophic lateral sclerosis, cystic fibrosis, dermatomyositis, Gaucher disease, hemophilia, multiple sclerosis, myasthenia gravis, Parkinson’s disease, polymyositis, rheumatoid arthritis, scleroderma, seizure disorders, sickle cell disease and systemic lupus erythematosus.
