IRS Releases Sample Forms for Employer Plan-to-IRA Rollovers

Pursuant to a mandate contained in the SECURE Act 2.0, the IRS has provided sample documents that retirement plan participants can use in order to complete certain rollover transactions. The forms are designed to be understood by the average plan participant, with the goal of simplifying and standardizing the rollover process to prevent confusion and errors. The forms are contained in Notice 2026-49 and are entirely optional. The forms are designed to apply to rollovers between employer plans and between employer plans and IRAs, but not to IRA-to-IRA rollovers. The forms take a check-the-box approach to guide account owners through the rollover process and ensure the plan sponsor has the information needed to complete the rollover transaction.
