The Ohio Department of Insurance has worked with the Ohio Association of Health Plans (OAHP) to create a standard health insurance application form for small groups.

The standardized form should simplify the process of shopping for small group coverage for employers and agents, Ohio department officials say.

The list of carriers using the form will include Aetna Inc., Hartford (NYSE:AET); Medical Mutual of Ohio, Cleveland; and UnitedHealth Group Inc., Minnetonka, Minn. (NYSE:UNH).

OAHP, Columbus, Ohio, will be keeping track of how the form works and may make suggestions for improvements later, the group says.
