New chronic condition templates could help health insurers customize plan benefits and incentives to suit the needs of individual enrollees with conditions such as diabetes or depression.

TriZetto Group Inc., Newport Beach, Calif., says it will work with the researchers who created the "value-based insurance" movement to develop the templates.

The templates will work together with the TriZetto Value-Based Benefits Solution health plan administration system.

In addition to diabetes and depression, the first set of templates will include templates for enrollees with asthma, coronary artery disease and smoking, TriZetto says.

The template for people with diabetes, for example, will decrease out-of-pocket costs for drugs, diabetes-monitoring office visits, and other services that can help improve control over diabetes, TriZetto says.

The researchers who will create the templates are Dr. A. Mark Fendrick and Michael Chernew of the Center for Value-Based Insurance Design, Ann Arbor, Mich.
