HHS To Award $60 Million In Health, LTC Navigation Grants

The U.S. Department of Health and Human Services will be helping states, area agencies on aging and other entities give more advice to seniors, people with disabilities, and caregivers.

The Affordable Care Act, the federal health system change package that includes the Patient Protection and Affordable Care Act and the Health Care and Education Reconciliation Act, provided funding for the HHS care navigation grant program.

The program is now distributing $60 million in grants, HHS officials say.

In addition to states and area agencies on aging, entities that can apply for the grants include State Health Insurance Assistance Programs and Aging and Disability Resource Centers, officials say.

Successful grant applicants might propose programs that can help “individuals who are under-served and hard to reach” learn about Medicare and Medicaid benefits, help older adults and individuals with disabilities find ways to stay in the settings of their choosing with the right supports, help people who are in hospitals and nursing homes return to the community, or strengthen connections between medical and social service systems, officials say.

Applications are due July 30.
