The MetLife Mature Markets Institute has published a 20-page guide for caregivers who are helping loved ones get health care.

The free guide includes a list of print and Web-based resources, a tool for collecting information for home health aides and other direct care providers, and a tool for collecting information for doctors, hospitals and other medical care providers.

The guide warns readers to make sure that they have a signed authorization form permitting health care providers to share medical information, according to the MetLife Mature Markets Institute, Westport, Conn., a unit of MetLife Inc., New York.
