A unit of Blue Shield of California has unveiled high-deductible preferred provider organization plans aimed at children and young adults.

The policies, the Essential plans 3000 and 4500, exclude coverage for normal maternity expenses and brand-name prescription drugs, but they include basic dental and vision benefits, according to California Blue, San Francisco.

California Blue, which is issuing the policies through Blue Shield of California Life & Health Insurance Company, says it will hold policy premiums to about $75 to $100 per month by imposing deductibles of $3,000 for the Essential Plan 3000 policy and $4,500 for the Essential Plan 4500 policy.

The out-of-pocket maximums will be the same as the deductibles, according to California Blue.

Co-payments for physician office visits are $40 per visit.

Insureds can get preventive care, dental care, vision care, 3 physician office visits per year and emergency care for fixed co-payments before meeting the deductible, according to California Blue.

Co-payments for generic prescription drugs from network pharmacies will be $10 per prescription, the company says.
