New program designed to reduce hospital admissions

Metropolitan Health Networks, a Florida healthcare services provider, and SeniorBridge, a national health management company, have announced a care management program to improve treatment outcomes and quality of life for Medicare beneficiaries at a reduced cost. SeniorBridge Care Management for Metcare will focus on care for patients with multiple chronic conditions.

The pilot program will offer nurses and social workers who provide in-home care for 100 Medicare Advantage members in Florida. Medical records for the patients will be coordinated via SeniorBridge’s Internet-based electronic medical record information system. The goal of the program is to reduce the number of hospital admissions experienced by chronic patients.

Eric Rackow, CEO of SeniorBridge, expects the program to be “a catalyst for improved care, to show significant return on investment and to be a healthcare reform standout.”
