Metropolitan Health Networks, a Florida healthcare services provider, and SeniorBridge, a national health management company, have announced a care management program to improve treatment outcomes and quality of life for Medicare beneficiaries at a reduced cost. SeniorBridge Care Management for Metcare will focus on care for patients with multiple chronic conditions.
The pilot program will offer nurses and social workers who provide in-home care for 100 Medicare Advantage members in Florida. Medical records for the patients will be coordinated via SeniorBridge’s Internet-based electronic medical record information system. The goal of the program is to reduce the number of hospital admissions experienced by chronic patients.
