Fort Dearborn Life Insurance Company is forming its first national group dental preferred provider organization insurance program.
Fort Dearborn, Chicago, a unit of Health Care Services Corp., Chicago, the same company that owns Blue Cross and Blue Shield of Illinois, says an affiliate has set up a PPO that already offers plan members access to more than 68,500 dental care providers throughout the United States.
