Some say Bill Clinton's health care reform didn't get off the ground because people were too afraid of losing their coverage. George W. Bush's Medicare expansion was eventually accepted, but the cost of it was all put on the deficit. The Patient Protection and Affordable Care Act was President Obama's attempt to integrate existing health structures into a system that can expand, but also control costs.
