When the Patient Protection and Affordable Care Act was signed into law in 2010, Jan. 1, 2014, seemed like a long time away, plenty of time for the law's opponents to overturn it with repeal efforts, Supreme Court challenges and defunding. But the key provisions of the law, including a Medicaid expansion, the individual mandate and cheap insurance for the poor remain. This sets the stage for a frantic 12 months of implementation throughout 2013.
