Washington state insurance regulators have imposed a $59,000 fine on an insurer over the insurer's use of a provider network contract for chiropractors that violated the state's insurance regulations.

Washington officials contend that the insurer, UnitedHealthcare Insurance Company, a unit of UnitedHealth Group Inc., Minnetonka, Minn., used an unapproved chiropractor provider contract after receiving a notice that the contract violated state insurance regulations.

UnitedHealth paid the fine in the fall of 2006 and now is using the approved contract forms, a company spokesman says.
