UniCare is making a Web-based personal health record system the core of its new Full Circle Health services package.
The package includes health promotion, wellness, health information, case management and care coordination services, according to UniCare, Chicago, a unit of WellPoint Inc., Indianapolis.
The personal health record will include UniCare claims data, the company says.
UniCare is offering the services to members enrolled in group health plans with 2 or more members.
Eligible members can use the services through the telephone, conventional mail or the Web.
